2nd Annual Baby- Crawling Contest Entry Form

Contest will be held in
Entry Fee-$5 per child
ALL PARTICIPANTS WILL RECEIVE A PARTICIPANTS GIFT BAG!!!
Entry Rules
1. Babies (entrants) must be between 6 and 12 months of age and not yet walking.
2. Entrants must be residents of
3. Entry forms must be received by Victoria Smith no later
than 10: am. Friday, September 17th, 2010.
4. Only one adult may accompany the baby on the “Race Track”.
5. Babies will crawl the length of the “Race Track,” which will be approximately 14’ in length.
6. At no time can the parent or guardian enter the boundaries of the “Race Track” without disqualification.
7. Should there not be a winner at the end of three minutes, the baby closest to the finish line will be
declared the winner.
8. Each baby is required to wear a disposable or regular diaper underneath their regular clothes.
